Electronic Funds Transfer (EFT)
Automated Clearing House (ACH) Authorization Form

Dear IUOE Local 501 Union Employer,

We are pleased to announce IUOE Local 501 Union Employers are now eligible to participate in our Electronic Funds Transfer (EFT) /Automated Clearing House (ACH) for immediate processing of monthly dues deductions.  This process reduces fraud and results in numerous benefits, including paperless and immediate transmittal.

The registration for EFT/ACH is direct pay.  This will allow the Employer to pay monthly dues deductions through on-line/Electronic download.  The Employer can set-up a one-time payment for dues deductions or monthly recurring payments once the initial documentation is complete.  If you, the Employer, are interested in setting up EFT/ACH direct pay for monthly dues deduction, please complete the Authorization Form attached.  

Please, remember to complete, sign, and return the Authorization Form to the IUOE Local 501 office in the envelope enclosed or you may Fax to 213.385.7324. If you have any questions, please do not hesitate to contact our office at 213.385.1561, extensions 128, 129 or 0.
